If your driver license, permit or ID card is lost, destroyed or becomes difficult to read, you may get a duplicate by applying in person at a DMV office. You may visit any office within the state regardless of your city or county of residence. See Fees.
Please bring identification documentation as outlined below. This will help confirm your identity and speed up the process. The Department may use your existing record and photograph currently on file when necessary.
Nevada uses a central issuance licensing system. You will be given a paper interim document and your permanent license or ID will be mailed to you.
Nevada residents out-of-state may also obtain a duplicate by submitting an Application for Nevada Driver License By Mail (DMV-204). Some duplicates issued by mail may not have your photo. You are required to surrender this "valid without photo" document for a photo license within 24 days of returning to Nevada.
Please bring at least one of the items below if you order in person. Use the Driver's License or Identification Card Application (DMV 002) and have it completed before you visit the DMV.
Documents must be valid originals or certified copies that were issued in the United States (except for foreign passports). We do not accept photocopies. Permanent Resident Cards must be updated at age 14.
You must present documentation of any name change(s) if you will be presenting a proof of identity document that does not contain your current legal name. This may include a Marriage Certificate, divorce decree, adoption records or court order. In a case of multiple name changes, you may be asked for records of each change.
If you lack these documents, contact your state or county Vital Statistics Office, www.vitalchek.com or a similar service and/or the Social Security Administration.
The Department is authorized to waive the $17 fee for homeless persons to obtain a duplicate Nevada driver's license or identification card. This is a one-time exemption for duplicates only. You must apply for this in person at a DMV office and submit the proper proof of identity and forms.
You may be required to reimburse the $3 photo fee to the Department at the time of your next regular renewal if you are employed at the time of the renewal.

The Department is authorized to waive the $17 duplicate fee for persons who have been released from prison within the past 90 days. You must supply official documentation of your release date from the Nevada Department of Corrections or its counterpart in another state.
